Yes. Bring the form your school or league requires and we will complete and sign it during the visit, so you leave with it done.
Most Florida schools and athletic associations require one each year. Check the specific requirement on your form, since leagues vary.
If we find something worth looking at, we will explain it clearly and tell you what happens next. Occasionally that means a referral before clearance. That is the physical doing its job.
